This is less a custom modification and more a piece of artistic craftmanship based on a cube. The designer modelled the 3x3x3 (probably a Fisher Cube) into the shape of a clownfish with a lot of amazing details. The red piece under the fish are made of actual rubber. Other designers would have modelled the fish's two sides equally but in this case the designer modelled a coral on the opposing side, creating the impression of a vibrant scene.
