A four times truncated pyraminx.
This is a transformation of the pyraminx. Here the pyraminx is truncated at all 4 corners. Each cut involves one of the 6 edges. This means that 2 of the edges remain untruncated. This technique of truncation gives the finished puzzle a special asymmetrical look although all faces look identical.
The images 3-4 show a version rebuilt by Mike Grimsley in 2006 which includes a beautiful custom made case.
