I'd never heard of that one. The research that I've done seems to indicate that it CAN happen, but should be very rare. It could involve lost sessions, timeouts, and similar techie stuff that failed for odd-ball reasons. I'm not going to do anything about it just yet, since I could be more likely to make things worse than to improve something that only seems to have happened once.
