I am not sure if I should be posting this here or at the Collector's forum,
so I wouldn't mind if it is needed to move. To the question:

Has anyone seen this puzzle, and if yes, does he/she know who made it?

Yes, that is correct. I have made a few hundred folding plate puzzles myself
(including prototypes, while most of them have a number of tiles which is not
a multiple of four), but clearly, the one in the photos above is not one of them.

I hope someone here knows who made it, so that the maker of this puzzle
will get the appropriate credit.
